Welcome to the Nicolet Medical & Dental Clinic (Northern Health Centers, Inc.) located in northern Oconto County on State Highway 32 in the town of Lakewood, WI.

The Nicolet Medical & Dental Clinic is an "FQHC" (Federally Qualified Health Center). As such, we are an entity serving a medically underserved population providing primary and preventative care services to Medicare, Medicaid, commercially insured, uninsured, and low income patients under the general or direct supervision of a physician.

We are dedicated to providing the best quality care to all of our patients, in a friendly and caring manner. Our clean, modern facility is pleasant and well equipped and our caring, professional staff is a well-trained team working together for each other and for the people of our community. We are a non-profit, consumer directed health care organization which provides high quality, cost effective health care, which is supported by federal grants under the United States Public Health Service Act. We meet rigorous federal standards related to quality care and services and cost of services, and we are qualified to received cost-based reimbursement under Medicaid and Medicare law.

Mission Statement Our mission at the Nicolet Clinic is to improve our community health by providing all persons a comprehensive program of quality family orented primary health care services regardless of their ability to pay, to all within our service area. Service Area The geographic area that we serve is defined by the Department of Health and Human Services and is an urban or rural area designated as having a shortage of health care professionals. The Northern Health Center's service area includes the Towns of Lakewood, Townsend, Wabeno, Mountain, Riverview, Doty, Silver Cliff, Athelstane, and parts of Langlade County as indicated by the gray area on the map below.
