Welcome to the Nicolet Medical & Dental Clinic (Northern Health Centers, Inc.) located in northern Oconto County on State Highway 32 in the town of Lakewood, WI.

The Nicolet Medical & Dental Clinic is an "FQHC" (Federally Qualified Health Center). As such, we are an entity serving a medically underserved population providing primary and preventative care services to Medicare, Medicaid, commercially insured, uninsured, and low income patients under the general or direct supervision of a physician. We are dedicated to providing the best quality care to all of our patients, in a friendly and caring manner. Our clean, modern facility is pleasant and well equipped and our caring, professional staff is a well-trained team working together for each other and for the people of our community. We are a non-profit, consumer directed health care organization which provides high quality, cost effective health care, which is supported by federal grants under the United States Public Health Service Act.
