in November from 10am - 5pm. The cost is $20.00 or have your
medicare card/number available. |

Nicolet
Medical & Dental Clinic offers a full range of programs and
services to meet the health needs of its clientele. Patients
living within our service area may qualify for our Sliding
Fee Scale based on family size and income level as determined
by Federal Income Poverty Guidelines.
Those on Sliding Fee Scale
are then eligible for the 340B pharmacy program where presciptions
are sold at government rates, a potential savings up to 50% of
retail cost. |
